Oncology Registered Nurse Navigator position in the Cancer Center Department in Norwalk, CT.

Full time position 40 hours per week, 8 hour shifts between 7:30am-5:30pm.

Norwalk Hospital is seeking a Self Starter for a Hematology Nurse Navigator Role in their Cancer Center

We are seeking a skilled and compassionate independent thinker to join our dynamic team of oncology nurse navigators. As a vital member of our outpatient cancer center, you will play a crucial role in delivering exceptional patient care through the navigation process guiding patients as they are diagnosed with a benign or malignant hematology diagnosis. We are hiring full time nurses to primarily work Mon – Fri but offer flexible hours between 8a- 630p. Experience in hematological diseases is required and certification by ONS is preferred but will consider applicants with the applicable experience and without the certificate/certification.

Summary

To crease a nurse navigator program at Norwalk Hospital. Role will be to keep patients in our system by navigating them to other sub-specialists and services.

Responsibilities

  • Conducts case findings. Screens cancer patients to identify support, education, discharge planning & referral. Comprehensive assessment of patient's physical, functional, psychosocial & financial status in cooperation with the health care team.
  • Develops, implements, and evaluates the plan of care and support through an interdisciplinary process in conjunction with the patient and family, linking the patient to appropriate institutional or community resources.
  • Procures services; serves as an advocate for patients and families with scarce resources. Facilitates referrals to state and local financial assistance agencies or programs.
  • Functions as a liaison between the hospital, physicians, patient, and community resources to achieve maximum communication between all parties so that care is coordinated and all needs of the patient are met.
  • Identifies quality issues related to cancer diagnosis, treatment and follow-up, risk management issues, and reimbursement issues with prompt referral to appropriate committee/personnel for action or resolution.
  • Assesses and evaluates cancer care through interaction with patients and surveys, and implements quality improvement projects as needed.
  • Serves as a representative of the cancer program/service line and lead person for coordination of care for patients.
  • Representing the cancer program provides education through health fairs and presentations to the community.
  • Supervises the volunteers within the cancer program, assuring compliance with appropriate patient interaction and confidentiality of information.
  • Participates as a member of the Cancer Committee; achieves the organization's established expectations with regard to customer service, teamwork, safety, and self-development.
  • Fulfills all compliance responsibilities related to the position.

Other Information

Bachelor's Degree required. RN CT Licensure Required. OCN required. Oncology Certification and prior experience a plus.
