Job Description

Job Summary:

Under the supervision of the South Bay program manager and Medi-Cal director, the Medi-Cal case manager will provide culturally competent, strength-based case management services to families with children and adolescents ages 6-20 who live within Santa Clara County (SCC) as part of CHC’s Medi-Cal clinic. Responsibilities include providing mental health and behavioral support to children and families with special needs, coordinating care, and supporting clinicians and psychiatric providers with following through with recommendations and linkages to resources. Case manager will support and assist families in accessing community resources that meet their individualized mental health needs and be part of an interdisciplinary team focused on behavioral and mental health needs for children and families. Services are provided primarily in the clinic but may include some community-based services.

Essential Functions:

  • Formulates, implements and monitors individual child/family development service plans in conjunction with treating team members
  • Supports families in following through with treatment recommendations made by therapists and psychiatric providers
  • Connects families to recommended resources and helps follow-through with recommendations made by team
  • Supports team in monitoring risk, as needed
  • Functions as primary liaison for family in linking to services and service providers at CHC
  • Assesses clients’ needs, strengths, and promotes healthy child/family development practices
  • Supports providers in completing Medi-Cal documentation and paperwork (e.g., CANS)
  • Collaborates with treatment team and links families to services within the county
  • Facilitates interagency transfers or transfers to other levels of care
  • Conducts follow-ups and monitors service delivery and client progress
  • Maintains confidential client records and files and completes required agency and Medi-Cal paperwork
  • Documents all chart contacts within 72 hours
  • Maintains 52% productivity (billable hours)
  • Administers evaluation surveys as required by contract
  • Understands and adheres to the laws governing child abuse reporting
  • Attends meetings and trainings necessary for professional development and as required by the agency or its funding sources
  • Performs other related duties as required and assigned

Knowledge/Ability/Skills:

  • Fluent in English and Spanish (verbal and written communication) required
  • Ability to qualify as a Mental Health Rehabilitation Specialist (MHRS) with the Mental Health Department
  • Effective verbal and written communication skills

Education/Experience:

  • BA/BS in psychology or related field from an accredited academic institution with course work geared towards child development and behavioral mental health
  • Require at least 4 years of experience in mental health settings serving children and families or adjacent field (requirement for MHRS credential)
  • Prefer two years of experience with children ages 6-20, including parental support and coaching techniques

Physical Requirements: An employee in this position may be required to occasionally lift, move and/or set-up stacks of documents, laptop computers, charts and other materials. Employee may also be required to work for extended periods of time on a laptop.

Special Job Requirements: To meet the needs of our clients, flexibility in work schedule is required. Most work will likely be in the afternoon/early evening.

Contact with Others: Frequent interpersonal interaction with peers, team members, leadership team, and outside contacts.

Compensation:

$28.85 to $38.46 per hour based on a 1.0 FTE. Position will start at 0.8 FTE.
